Cheek slimming and face slimming in Vienna
Buccal fat removal (bichectomy)
Causes of "chipmunk cheeks"
The causes of full cheeks can be both genetic and age-related. Some people naturally have full cheeks, while others may have excess fat in the cheeks due to weight gain or other factors. These fat deposits cannot be reduced by diet and exercise.
Cheek slimming and face slimming
Bichectomy, also called buccal fat removal, is a minimally invasive surgical procedure used to remove excess fatty tissue in the cheeks. The operation is an effective way of giving the facial contours a slim, attractive shape and fulfilling the wish for a slimmer, more defined appearance.
Many people suffer from so-called chipmunk cheeks, which make them look older and heavier. However healthy and slim you may be, these chubby cheeks often remain and reduce self-confidence.
Bichectomy is a procedure that aims to remove these bothersome fat deposits below the cheekbones. The procedure is minimally invasive and is usually performed as an outpatient, which means you can go home the same day. The whole procedure usually takes no longer than an hour and is performed under local anaesthesia.
During the operation, Dr. Handl makes a small incision inside your mouth and removes the excess fatty tissue from the inside. Once the fatty tissue has been removed, Dr. Handl closes the wound with a fine thread that dissolves by itself after a few days. You can go home immediately after the procedure and continue your normal activities. Note, however, that swelling and bruising can occur for a few days. These disappear quickly, and the result of the operation gradually becomes visible.
Bichectomy has proven to be a safe and effective method of achieving a slimmer, more defined face. The results are usually permanent and can boost your self-confidence considerably. Careful aftercare and consultation are just as important, however, to achieve an optimal result.
Dr. Handl is an experienced specialist in plastic and aesthetic surgery with extensive experience in bichectomy. In the consultation, he will inform you in detail about the risks and course of the procedure and answer all your questions. If needed, he will also suggest other options for your treatment in order to achieve the best possible result.

A small operation with a big effect
Bichectomy, also known as buccal fat removal, is a minimally invasive surgical procedure in which the excess fat deposits in the cheeks are removed. Dr. Handl performs the procedure in Vienna as an outpatient, and it usually takes about an hour.
Before the operation, there is a detailed consultation in which Dr. Handl explains the course of the operation as well as the risks and possible complications. He also discusses the patient's individual wishes and ideas and the results to be expected.
On the day of the operation, the patient receives local anaesthesia. Dr. Handl makes small incisions in the mucous membrane of the mouth to access the fat pads. He then removes the excess fat and closes the wound with fine stitches.
After the operation, the patient is observed at the practice for a few hours to make sure there is no secondary bleeding or other complications. We recommend eating soft food for a few days after the operation and not smoking.
Most patients can return to their normal activities as early as the next day. It is advisable, however, to avoid physical exertion for a few days and to clean the face gently. After about a week, the stitches are removed and the final result of the bichectomy becomes visible.
Dr. Handl recommends regular check-ups to make sure healing proceeds normally and that the result is lasting.
Surgical method
Buccal fat removal (buccal lipectomy) is performed as an outpatient under local anaesthesia. A small incision is made inside the mouth.
Through this incision, the cheek fat is exposed and then removed (partly or completely). The mucous membrane is then closed with dissolvable stitches.
Preparation, risks and aftercare
Preparation
In the consultation, Dr. Handl checks whether full cheeks really come from the buccal (Bichat) fat pad and whether removal suits your face and age. Because the face naturally becomes slimmer over the years, fat is removed conservatively. Have your teeth and gums checked beforehand if there is any inflammation.
The procedure is performed as an outpatient under local anaesthesia through the mucous membrane of the mouth. After consulting us, avoid blood-thinning medication such as aspirin, as well as alcohol and, if possible, nicotine, which delays wound healing.
Risks
The procedure is performed through a small incision in the mucous membrane of the mouth, so there are no external scars. Swelling, bruising and infection are possible. Rarely, branches of the facial nerve or the duct of the salivary gland are injured. Because the face naturally becomes slimmer with age, removing too much fat can later lead to hollow cheeks, which is why Dr. Handl removes fat conservatively.
In the informed-consent consultation, Dr. Handl discusses all risks in detail and in relation to your personal situation.
Aftercare
After a short rest, you can go home. In the first week, soft food and an antiseptic mouth rinse after every meal support the healing of the mucous membrane. Cool your cheeks in the first days and sleep with your upper body raised.
Office work is usually possible the next day. Avoid alcohol, nicotine and sport for two weeks. The cheeks are swollen at first; the new contour shows after a few weeks.
Frequently asked questions
When is buccal fat removal advisable?
Buccal fat removal is usually the appropriate method for round, chubby facial features that are felt to be bothersome. So-called chipmunk cheeks often make you look more childlike or overweight, even when you are not overweight. In these cases, removing the pronounced fat deposits is suitable for contouring your facial features.
How does buccal fat removal change your appearance?
Removing the fat deposits makes the facial features look slimmer and more contoured. No visible scars remain.
What are the risks of buccal fat removal?
Buccal fat removal is considered a very low-risk treatment. Immediately after the procedure, swelling can occur, but it subsides within a few days. In your individual consultation, you will be informed in detail about all the options, limits and any risks of the procedure and have the opportunity to clarify any open questions.
Will there be visible scars after buccal fat removal?
Because the fat is removed through a small incision inside the mouth, no scar is visible on the outside. The incision in the mucous membrane is closed with dissolvable stitches and is usually no longer visible after a few weeks.
Is the procedure painful?
The procedure is performed under local anaesthesia and is therefore usually not felt to be painful. If you wish, twilight sedation may also be possible.
How long does the procedure take?
The procedure takes between 30 and 60 minutes. Afterwards, you can go straight home.
When can I go out in public again after buccal fat removal?
Please note that on the first day after the procedure you should only have liquid food so as not to disturb wound healing in the mouth. Swelling can still occur in the first week after buccal fat removal. Apart from that, you can go out in public again straight after the procedure.
What do I need to consider before the procedure?
Please avoid medication that affects blood clotting, such as aspirin, for 14 days before the procedure. You should also largely avoid alcohol and cigarettes, as these can impair wound healing.
What do I need to consider after the procedure?
For optimal wound healing in the mouth, soft food and regular antiseptic rinses are recommended in the first weeks after the procedure. Alcohol, nicotine and sport must be avoided for the first two weeks after the procedure.
When can I do sport again after the procedure?
We recommend avoiding sport for the first 2 weeks after the procedure.
What are the cancellation terms?
Free cancellation of the operation is possible up to 4 weeks beforehand. From 2 days before the operation, cancellation is only possible with payment of 100%. Rescheduling is always possible after consulting the team.
