Breast implant removal
Capsular contracture, rupture, aesthetics or a personal decision
For many women, implant removal can be a necessary step to address both aesthetic and medical issues. If you are interested in implant removal, it is important to know that there are many reasons for this procedure. Aesthetic reasons can arise from natural ageing, hormonal fluctuations or major weight gain or loss. But medical reasons such as an infection, a rupture of the implant or capsular contracture can also make removal or exchange of the breast implant necessary.
Capsular contracture can lead to hardening or a change in breast shape and is often associated with pain. If you feel uncomfortable because of a problem implant, implant removal can be the best choice to relieve your discomfort and return your breasts to their natural shape.
Dr. Handl has many years of experience in treating problem implants and will take care of your concerns. In a no-obligation conversation, you can discuss your options and see Dr. Handl's expertise in breast surgery for yourself.
It is important to emphasise that the way to beautiful, natural breasts often requires patience. Dr. Handl is an experienced specialist and accompanies you on your way to a perfect result. Implant removal can help relieve your discomfort and restore your natural beauty. Please contact us to find out more about the options for implant removal.
Removal of breast implants
The course of a breast implant removal depends on various factors, such as the type of implants, their size and the condition of the breast tissue. In general, the operation can be performed under local or general anaesthesia and takes between 30 and 120 minutes, depending on its complexity.
At the start of the operation, an incision is made to remove the implant from the breast. Depending on the position of the implant, the incision can be made in the breast crease, around the areola or under the arm. The implants can be removed directly from the breast, or the capsule can be removed along with them to ensure that the implant is removed completely. In some cases, it may be necessary to lift or reshape the breast tissue or skin in order to achieve an aesthetic result.
After the implants have been removed, the incisions are carefully stitched and a dressing is applied. In most cases, patients can leave the clinic the same day but need to be careful and take it easy in the days after the operation. Painkillers and antibiotics can be prescribed if needed.
It is important that patients attend regular follow-up examinations after the operation to make sure the breasts are developing healthily and no complications occur. In some cases, a further breast reconstruction or lift may be recommended to achieve an aesthetic result and strengthen the patient's self-confidence.
Implant removal: the day of surgery
On the day of your treatment, we ask you to arrive at the clinic at least 4 hours before the operation. This allows you to check in calmly at the private clinic, as in a hotel. After the usual admission procedures, you are taken to your room and the anaesthetist goes through all the details of the anaesthesia with you once more.
Directly before the procedure, you are measured and photographed again, and careful documentation is prepared. You can voice wishes and ask questions at any time. Dr. Handl never operates under time pressure, so the operating time varies with the surgical technique, your anatomy and the effort involved.
After the operation, you spend the first hours in the recovery unit. Experienced nursing staff look after you constantly until you feel fit enough for your own room. Once you are back in your room, you can also receive private visitors.

Preparation, risks and aftercare
Preparation
Please bring your implant passport and, if available, previous surgical reports to the consultation. Dr. Handl discusses with you whether only the implant or also the capsule will be removed and what your breasts are likely to look like afterwards. If you would like a firmer shape, a breast lift at the same time or later may be advisable.
Before the procedure, a health check and, if necessary, an anaesthesia briefing take place. From the age of 30, Dr. Handl recommends a precautionary breast examination with ultrasound or mammography before any breast surgery. After consulting us, avoid blood-thinning medication such as aspirin, as well as alcohol and, if possible, nicotine, which delays wound healing.
Risks
As with any operation, bruising, swelling, secondary bleeding, infection and wound-healing problems can occur.
After removal, the breasts may look smaller and softer, and depending on its elasticity, the skin may not retract completely. If the capsule is also removed, the risk of bruising and fluid collections increases. Changes in nipple sensation and irregularities in contour are also possible, which may make a later lift advisable.
In the informed-consent consultation, Dr. Handl discusses all risks in detail and in relation to your personal situation.
Aftercare
The removal is usually performed as an outpatient procedure, so you can go home the same day. Office work is often possible again straight away. Wear the sports bra day and night for four weeks; it helps the tissue settle. Do not lift anything over 5 kg in the first weeks and do not sleep on your front for two weeks.
The first check-up takes place after about one week, and the stitches are removed after about 14 days. It takes a few months for the skin to adapt to the new shape of the breasts. Protect the scar from the sun.
Frequently asked questions
When is breast implant removal performed?
On the one hand, there can be aesthetic reasons for implant removal, such as dissatisfaction with the appearance due to changes caused by ageing, weight gain or loss or hormonal fluctuations. On the other hand, medical reasons can make an implant exchange or removal necessary, for example if an existing implant has led to complications such as capsular contracture. With old breast implants, switching to a modern implant or removal is also often advisable for medical and aesthetic reasons.
What are the risks of breast implant removal?
Like any surgical procedure, breast implant removal carries risks, for example wound-healing problems or scarring. In the consultation, Dr. Handl informs you in detail about all the risks of the procedure.
How does implant removal work?
The treatment is usually shorter than the first insertion, and the procedure can be performed under either local or general anaesthesia. First, a fine incision is made on the breast in order to remove the existing implant intact and gently. This is usually and preferably done at the same place where the implant was originally inserted, so removing the breast implants does not cause any additional scars. Once the removal is complete, the incision is stitched and the treated breast is supported with a dressing.
Does breast implant removal cause additional scars?
Care is taken to make the incision at the same place where the implant was originally inserted, so removing the breast implants does not cause any additional scars. Dr. Handl also pays particular attention to keeping scars inconspicuous.
What can I do to help the scars heal well?
Dr. Handl pays particular attention to keeping scars inconspicuous. For optimal healing, we also recommend using scar care products and protecting the scar from direct sunlight.
What do I need to consider before the procedure?
Before an operation, it is important to avoid medication that affects blood clotting where possible (e.g. aspirin). You should also largely avoid alcohol and cigarettes. Nicotine can reduce the blood supply to organs and delay wound healing, so we recommend giving up cigarettes as far as possible during the treatment phase. You will find out more in your consultation with Dr. Handl.
Is the procedure painful?
The procedure takes place under local anaesthesia with twilight sedation or under general anaesthesia and is therefore usually felt to be hardly painful. In the first days after the procedure, pain can occur, which can be managed with painkillers. Pain perception varies from person to person.
When can I do sport again after breast implant removal?
You should avoid sport for the first 2 to 3 weeks after the procedure.
How long will I be off work after the procedure?
That depends on your job. We recommend taking at least 1 to 2 days to recover after the procedure. Desk work is generally possible again the next day.
How can I support the healing of the scars?
Dr. Handl pays particular attention to keeping scars inconspicuous. For optimal healing, we also recommend using scar care products and protecting the scar from direct sunlight.
What do I need to consider after the procedure?
After the operation, you recover during a rest period in the pleasant surroundings of our premises. Dr. Handl goes through everything necessary for your discharge with you, and you can leave the clinic the same day. Physical rest is very important after the operation. You also need to wear a special dressing or bra. Please avoid movements that can cause strong jolts, tension or pressure in the chest area, and avoid lifting heavy objects. You should also avoid sleeping on your front for the first two weeks. The wound should be protected from water for the first 14 days. The stitches are removed after about 14 days.
